If you experienced a problem with one of the venders, please fill out and submit the following Problem Report Form

If what you want to tell us is really more of a suggestion, Registration or a request for enhancement, please use our suggestion form instead.

Problem Report Form

When you submit your Delivery Report, it will be entered into our database for tracking purposes. It will be associated with the account name under which you accessed this web. After an initial assessment by our support department, the resolution of the problem will be assigned to an appropriate Personnel. Periodically we will post notices regarding Delivery Problem reported by our members in the Delivery Problems section of this page, and tips or workarounds on the FAQ page.

Please be as specific as possible when describing the problem.

  3. Location Address ? 
  4.  Igsoa Membership ID:

  5. Please Specify the Truck # ?
    Name of the Delivery Company:
    Time of Delivery ? Delivery Date ?
  6. (example: "Late delivery more than one hour")
  7. If the problem need more details, give the details:

    (example: "1. Over/Less charged. 2. less/more product delivered ...")

    If the problem is not reproducible (only happened once, or occasionally for no apparent reason), please describe the circumstances in which it occurred and the symptoms observed:

    (note: Its hard to fix if we are not reported every time it occure.)

Known Problems

The following is a list of some common problems with most of the venders; we are working hard to correct them and get you the best possible service all the times.

  • Delayed or Late delivery without notice #1.
  • Order Cancelled or Product not delivered at all without any notification #2.
  • Products supplied from different terminal without approval #3.
  • Price charged less or more than agreed upon #4.
  • Late delivery credit not applied in next invoice #5.
  • No answer from the supplier for order follow up #6.
  • For delayed Purchase Order, No delivery status available  #7.
